Gavi’s Role in Polio Eradication Combining Forces with IPV

Gavi, the Vaccine Alliance, stands at the forefront of global health initiatives, steadfast in its commitment to universal immunization. The organization, renowned for vaccinating over half of the world’s children against life-threatening diseases, has opened avenues for eligible countries to propose the integration of diphtheria, tetanus, and pertussis (DTP) boosters. Simultaneously, nations meeting Gavi’s eligibility criteria now have the opportunity to transition to the hexavalent vaccine, a revolutionary six-in-one immunization combining key vaccines, including diphtheria, tetanus, whole-cell pertussis (DTwP), hepatitis B, and Haemophilus influenzae type b, with inactivated polio vaccine (IPV).

This strategic move aligns seamlessly with Gavi’s overarching goal of achieving universal immunization, addressing the global burden of common childhood illnesses caused by these pathogens. Annually, these diseases collectively claim approximately 110,000 lives, with Asia and sub-Saharan Africa bearing the brunt, especially among children aged one to four years. The introduction of DTP boosters emerges as a pivotal measure to safeguard children who have missed routine vaccinations, are under-immunized, or reside in underserved communities. The broader objective is to expand immunization coverage, preventing recurrent outbreaks of vaccine-preventable diseases and thereby enhancing overall health and developmental outcomes.

The Gavi Board’s approval of the hexavalent vaccine in June 2023 marks a significant leap in vaccination strategies. This comprehensive six-in-one vaccine is anticipated to streamline the delivery of protection against all six diseases, contributing to more efficient vaccination efforts. The integrated approach of combining IPV with pentavalent vaccines supports the global initiative for polio eradication, providing increased opportunities for under-immunized children to receive sufficient IPV doses. The incorporation of IPV into a combination vaccine with other core antigens ensures a seamless integration into national immunization programs, aligning with the broader vision of transitioning toward a post-polio world.

Thabani Maphosa, the Managing Director of Country Programmes Delivery at Gavi, underscores the importance of continually expanding the vaccine portfolio. Immunization, as the most widespread routine health intervention, reaches more communities than any other health program. Despite this, significant challenges persist, with too many un- and under-immunized children globally. The ongoing COVID-19 pandemic has exacerbated these challenges, emphasizing the urgency and importance of Gavi’s mission.

Gavi’s commitment to expanding its vaccine portfolio is part of a broader strategy outlined every five years. Despite delays caused by the pandemic, Gavi, in collaboration with its partners, is gearing up to introduce additional vaccines, including the hepatitis B birth dose and rabies vaccine, in 2024. The organization is actively engaged in developing the 2024 Vaccine Investment Strategy, a comprehensive plan undergoing a rigorous process to identify new vaccines or candidates with the most significant impact on lower-income countries. The upcoming Gavi Board meeting in Accra, Ghana, will provide a platform to discuss the shortlisted candidates and further refine the strategy.
